A Coventry housewife died in a crash on the M40 that left two others critically injured, including a baby boy.

The cause of Zamir Akhtar’s death was confirmed following a post-mortem examination to be multiple injuries, Oxford Coroner’s Court heard as the 65-year-old’s inquest was opened this week.

Mrs Akhtar, from Coventry, was reported to have been involved in a collision on the M40 northbound near junction nine on the morning of May 18.

According to coroner's court documents, a South Central Ambulance Service paramedic verified her death at 9.56am that morning.

At the time, Thames Valley Police said two other people, including a one-year-old boy, had been taken to hospital with critical injuries.

Their car, a grey Nissan Qashqai, was said to have left the northbound carriageway of the M40 between Bicester and the Cherwell Valley Services at speed.

Of the five occupants in the Nissan, a man in his 30s and a two-year-old girl suffered less serious injuries.

Sgt Dominic Mahon of the Serious Collision Investigation Unit, said: “I am appealing to anybody who was driving on the M40 at around the time of this collision to please get in touch with Thames Valley Police if you have witnessed this incident.

“I’d also ask anybody who was on the M40 between junctions 9 and 10 who has dash-cam to please check this and get in touch if it has captured the collision or the moments leading up to it.”

A full inquest into Mrs Akhtar’s death will take place on November 23 at Oxford Coroner’s Court.
