A HEALTH authority has been disciplined for two information leaks, including accidentally publishing 4,200 people’s personal data online.
Watchdog the Information Commissioner (ICO) told Oxford Health NHS Foundation Trust to improve some of its processes for protecting private information.
One file leaked included the email addresses, user names, passwords and billing addresses of 4,200 people.
It was accidently published in May 2013 while a new website was being created for the Oxford Centre for Cognitive Therapy.

In another incident, in January 2013, a letter containing someone’s mental health information was sent to a wrong address.
The ICO has now served Oxford Health with an ‘undertaking’, which has been signed by trust chief executive Stuart Bell, setting out the action the trust must take.
Stephen Eckersley, the ICO’s head of enforcement said: “Both these incidents were down to human error. Organisations must have contracts, procedures and plans in place to reduce the risk.”
Oxford Health NHS Foundation Trust spokesman Alistair Duncan said: “The trust now has in place stronger systems to prevent this occurring again. We have also apologised to the service user whose letter was sent to the wrong address.”
