Age UK Oxfordshire has been given £225,000 by the Government to help support the elderly and tackle the issue of bed-blocking.
Bed-blocking is when patients are ready to leave hospital but cannot because community services are not available.
The Circles of Support project will involve using volunteers, healthcare professionals and adult social care workers to provide help for the elderly to prevent them from needing to go to hospital.
